i could also go with Sam but John did put a heavy load on Dean's shoulders with the Sam he pretty much raised him & then his dad told him to either save Sam or kill him i don't know about you but i could never handle either of my parents telling me i would possibly have to kill one of my sisters it just wouldn't happen i do it the same way as Dean did if i had to.

The weight he put on Dean's shoulders and the reliance the two of them had on one another, especially after Sam left for Stanford, let's face it, probably wasn't healthy. Dean looked up to him so much and John put so much on him that they kind of gave a received things from each other on a two-way street. So yeah, Dean was pretty badly damaged - especially after he died.
