But i think it relates to the patiens POV theme to cause there really is no talking so to know what the POTW is thinking it kinda has to be in POV of the POTW rii?

This one's new AH. I do remember a casting call for someone that can do sign language a while back, but not one requiring a young deaf actor/actress.

I would assume they're going to be in the same episode, but I'm really interested in what the plot's going to be for it.
