Stupid question, but what did were Derek and Meredith doing in the elevator?

if toi are talking about the lastest episode, they were so call going on their date. their rendez-vous amoureux, date was supposed to be sex in the on call room but bailey was in there. but because Meredith only had a minute to spare and the room was occupied, Derek decided on something else, they went into the elevator where Meredith thought Derek wanted to have sex there but instead he gave her a fertility shot.
