Angel Beats! Question

One thing I'm wondering about...

Didn't Otonashi's sister end up dying? How come she didn't end up in the Afterlife...? I thought maybe she was satisfied with her life, but how could she be if she died so young and didn't get to grow up with Otonashi at her side? I suppose I'm kind of Lost on this one.
